Setting Up the Plan for Your Business

We believe signing up for workers’ compensation managed care plan should be easy. Whether you’re a new customer or old friend, this straightforward process is the same.

STEP 1

Download and review these guides for employers and employees. They explain The Hartford’s workers’ compensation and simplify your setup.

These packages include an acknowledgement form that will be filled out in Step 2.

STEP 2

Once your employees have reviewed their enrollment package listed in Step 1, have them sign the workers’ compensation network acknowledgement form on the package’s last page.

This form should be returned to you and retained for your records.

STEP 3

For New Hires

At time of hire, provide new employees with the Employee Enrollment Package and have them sign out the acknowledgement form.
